APPOINTMENT TIME

Appointment times are based on the time discussed with Library staff. This may differ from the time listed in the confirmation emails.

 

  1. Completed DS-11 Form
  2. Other forms if applicable (ie lost passport, etc.)
  3. Previous passport (if applicable)
  4. Driver License
  5. Check or Money Order to be made out to U.S. Department of State
  6. Passport photo
  7. Birth Certificate
  8. Other documentation as required
  9. Child is required to be in attendance
  10. Parents of minors are required to be in attendance
  11. $35.00 Cash, Charge, Debit card for Town and Country Library
